What Do I Do if I Lose My Medical Marijuana Card?

Have you lost your medical cannabis card? Don’t worry. It isn’t very fun but it probably isn’t the end of the world! Most medical marijuana programs allow you to get a replacement card if you have had yours lost or stolen. The challenge is, every state is different. Here’s what to do when you lose your medical cannabis card in each state.

Please note, we have only included states where Leafwell currently offers services and where the patients can have a physical copy of their card. For states where the certificates are digital, cards can never be lost — just remember to carry your cell phone with you when you go to purchase your medical cannabis products.

Arizona

Arizona issues electronic cards, which can be downloaded and printed directly from your state account.

Arkansas

If you submitted your Arkansas state application online, you can print a replacement through your online state account. If you don’t have an online account, you can call the Arkansas Medical Marijuana program at 501-682-4982 and request a replacement card.

California

You don’t technically need a card in California, your PDF of your Leafwell certificate is enough. But if you do misplace your Leafwell certificate or card, just contact us and we can send you a replacement for $20.

Colorado

Your card is emailed to you if you apply online in Colorado. You can then print out the card. Therefore, if you lose your print-out, you can just print a new one. We recommend you save the original email somewhere in your inbox but if you don’t, you should be able to recover it from your deleted mail.

Connecticut

Connecticut state charges $35 for a replacement medical marijuana card. Fill out this form to get a new card.

Delaware

Delaware state charges $20 for a replacement medical marijuana card. Mail a request for a replacement and the $20 fee to:

Florida

Florida state charges $15 for a replacement medical marijuana card. Fill out this form and provide a copy of your Florida Driver’s License or ID card.

Georgia

Georgia state charges $30 for a replacement medical marijuana card.

You can contact the state by email (THCRegistry@dph.ga.gov) or phone call (770-909-2765) to request a replacement. The state will verify the information with your doctor, and then a new card will be shipped.

Illinois

Illinois no longer issues plastic cards. You simply download and print out your digital card. There are instructions here.

Hawaii

You can download your medical cannabis card directly through your Hawaii state account.

Louisiana

There are no medical marijuana cards in Louisiana. However, you can request a printout of your faxed certificate from your dispensary.

Maine

Leafwell can send a replacement Maine medical cannabis certificate for $20. Just contact us.

Maryland

Maryland issues digital cards by default, and ordering a physical card is optional. If you need to replace a lost or stolen card, you need to log into your account to report the missing card, then pay $50 for the replacement.

Massachusetts

Massachusetts state charges $10 for a replacement medical marijuana card. Contact the state to request a replacement on 833-869-6820.

Michigan

You can request a replacement card using an online account in Michigan or submit a form by mail. There is no fee for this replacement in Michigan.

Minnesota

There are no medical marijuana cards in Minnesota. You can, however, download a Registry Verification Document from your state account.

Missouri

Missouri doesn’t issue physical medical marijuana cards. You can download your card at any time through your state registry account.

Montana

Montana state charges $10 for a replacement medical marijuana card. Complete the ‘Report Lost Patient Card’ application within the TransAction Portal.

Nevada

You can request a replacement card by emailing MMRegistry@health.nv.gov. There are no replacement fees mentioned.

New Hampshire

New Hampshire state charges $25 for a replacement medical marijuana card. Fill out this form.

New Jersey

New Jerseyno longer issues physical cards. You can download and print your card directly from your state account.

New Mexico

Download your medical marijuana card through your New Mexico state account online.

New York

You can log in to your Leafwell account and download your certificate from your patient dashboard.

Ohio

You can access your medical card online at any time in your Ohio Medical Marijuana Registry account.

Oklahoma

Oklahoma patients and caregivers can request a replacement through their Thentia portal – log into the new portal and use the navigation menu under “Licensing” to select “Request a Replacement Card.” If you never received an email with your new login details, contact servicedesk@omes.ok.gov.

Oregon

Oregon charges $100 for a replacement medical card. The fee is reduced to $20 with proof of SSI or veteran status. The fee will be waived for veterans who provide proof of a 50% or more disability rating.

Make sure to include your name, mailing address, and whether the replacement is for a patient, caregiver, or grower card.

Pennsylvania

Pennsylvania state charges $25 for a replacement medical marijuana card. If you lose a card more than once, you’ll be charged $50 for the second, third, or more replacements.

Request a replacement under the ‘profile settings’ tab of your account on the Pennsylvania website.

Puerto Rico

Puerto Rico doesn’t issue physical cards, so you can log in to your Puerto Rico online account and download your card.

Rhode Island

Rhode Island state charges $10 for a replacement medical marijuana card.

Texas

There are no physical medical marijuana cards or certificates in Texas.

Vermont

Request a replacement card online through the patient portal, or contact the Vermont Medical Cannabis Program by phone at 802-828-1010 ext.2 or email CCB.Med@vermont.gov. There is no fee for replacements.

Virginia

You can view and download your certificate in your state account in the Account Dashboard tab.

Washington

Leafwell can send a replacement Washington medical cannabis certificate for $20. Contact us here.

Washington DC

Washington DC state charges $10 for a replacement medical marijuana card. Fill out this form and email it to ABCA.CannabisInfo@dc.gov, or mail or drop it off to:
